Insurance & Payment Policies

Insurance
As a licensed professional counselor, my services should be covered in whole or part by your health insurance or employee benefit plan. I currently accept Blue Cross and Blue Shield, Aetna, and Cigna. Your co-insurance or co-payment is due at each session. For all other insurances, I will bill you directly and give you a statement with the information needed to file directly with your insurance company. Please check your coverage carefully by asking the following questions:

  • Do I have mental health insurance benefits?
  • What is my deductible and has it been met?
  • How many sessions per year does my health insurance cover?
  • What is the coverage amount per therapy session?
  • Is a referral required from my primary care physician?

Payment
Cash, check, credit/debit cards, and FSA/HSA benefit plans are accepted for payment.

Cancellation Policy
I require 24-hour notice if you need to cancel your appointment. If you do not provide such notice, you will be charged a $75 cancellation fee.

Good Faith Estimate Notice
You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services.

You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.

For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises.
